Downloading Instructions for pdf files

Our preference is to have you download all forms as PDF (Adobe Acrobat) files because they will print out as exact duplicates of the forms that were mailed. Please follow the instructions below to download the forms in PDF format.

Step 1. Determine if you have Adobe Acrobat Reader. If not, please download the reader from the following site Download Adobe Acrobat Reader

Step 2. If you have Adobe Acrobat Reader installed on  your computer, download the form by clicking on the (pdf link) for the form that you need. 

Step 3. Print the form by clicking on the printer icon in the Acrobat Toolbar. Note, do not print from your browser's print button.
